Some people are wondering if he is actually a demon, but I think the line about, "It'll turn me into a demon!" was sarcasm on Dean's side, the kind of, "Duh, ya think!" sarcasm that he's known for.
The thing that worries me most is if and when they get him back, how hard will this hit him, the killings (self defence or not) etc...
